Special Tools Developed for Use in the Sponsorship Program • The "Prayer Garden Project" computer program - This is a complex Access-based database program designed to support all aspects of the Prayer Garden Project, including the Sponsorship Program. • "Intent to Sponsor" forms - These forms are used to gather all of the needed information for each sponsorship.  A copy of the form must be completed by the sponsor for each item being sponsored. All information contained on the form will be entered into the "Prayer Garden Project" computer program.  This includes: • Sponsor contact information  Name  Address  Phone Number  email address (optional) • Information describing item to be sponsored  Olive Trees and Benches – Each sponsored tree or bench includes a bronze plaque with the exact wording provided by the sponsor on the “Intent to Sponsor” form. • Sponsorship Bricks – The information necessary for sponsored bricks is more extensive than for an olive tree or bench, and includes:  Size of brick (4"x8" or 8"x"8")  Format option for inscription - defines the number of lines of text, the actual wording of the text, and for some options, specifies inclusion of an image of drawing of a cross or dove and the position of the image relative to the lines of text.  Inscription text to appear on the brick  Which image is to be included (cross or dove) • "Internal User Only" Information section - this information is to be entered by a church office worker on the form at the time that the form and payment are submitted.  Including:  Date payment made  Type of payment (cash, check, or credit card)  Amount of payment  Check number or last 4 digits of credit card (if applicable)  Receipt number - number on receipt given to sponsor A sample of an “Intent to Sponsor” form is shown here. • A Specially-Designed, Computer-Generated "Thank You" Card - A card will be mailed to each sponsor for every item sponsored.  The “Thank You” card will contain a thank you message with the details of the sponsorship item and payment and also include a computer-generated “proof” image of the sponsored item for the sponsor to examine and approve or revise.
